London tower block fire: What went wrong at Grenfell Tower?

Investigators are trying to establish how a fire in a Londontowerblock was able to spread so aggressively and so fast.

A renovation of the building was completed in 2016. Cosmetic cladding was added to the outside, with some experts suggesting it could have acted as an accelerant.
